This page includes links to two pages to download state approved special education forms for use by local school districts.
The first page Special Education Forms - Due Process, contains downloads to special education forms commonly used by local school districts to document due process and implementation of appropriate programs. Included are state approved forms for the Referral, Consents, Individual Education Program (IEP), Conference Summary and other state approved special education forms.
The second page, Special Education Forms - Eligibility Determination contains downloads to special education forms used by local school districts to document eligibility for special education. Included is the Consent to Evaluate, Evaluation Planning Form, and each of the Eligibility Determination forms specific to the suspected disability.
These forms will be available in the state's student information system (SIS) during Summer 2011. If a form is not currently available in the SIS, the Microsoft Word version of the forms on the linked pages can be completed and uploaded at the student level.
Revisions to the forms in the SIS occur based on feedback the Division of Learning Services receives from Director’s of Special Education, State Stakeholder Groups, KDE Interpretation of Law, and Legal Decisions. In addition, the Division of Learning Services, Diverse Learners Branch make revisions to the state mandated forms based on on-site and desk audit monitoring.
